Scheduling
Our office hours are Monday through Friday from 8:00am to 4:30pm. We will schedule your first appointment as promptly as possible. If you are in discomfort or require emergency dental care, every attempt will be made to address your dental need as quickly as possible.
In order to expedite your consultation visit please download and fill out all required forms and bring them to your 1st visit.
If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to dental treatment (e.g. artificial heart valves or joints, heart murmurs requiring premedication, diabetes, hypertension, or pregnancy.)
If any dental radiographs have been taken within the past 2 years, please request that they be forwarded to us or provide us with the name of the dentist and we will request copies.
